This suggests that their capacity to generate an elevated red cell count may be limited by physical and genetic factors. Further clinical analysis of large numbers of ET patients defined by their JAK2V617F status from MRC PT1 compared with that from PV patients is underway to assess whether there is a true clinical difference between these disorders in terms of disease related events. A recently published small case series comparing 179 patients with ET and 77 with JAK2V617F positive PV demonstrated a gradient in various laboratory values between patients with JAK2 wild-type ET, JAK2V617F ET and PV. Furthermore the rate of thrombotic complications in JAK2-positive ET patients was significantly higher than that in wildtype ET patients and not statistically different from that in PV patients.19 Although this data supports a common clinical phenotype for JAK2V617F positive ET and PV a significant underlying biological difference between these conditions was identified by Scott and colleagues.20 They reported the results of carefully carried out colony.
